The appointment wasn't attended in the time and date specified and scheduled. Take into account that appointments should be cancelled beforehand if the patient is not able to attend.
If no further comments or desire to reschedule are expressed after 48 hours, you may have to send a new appointment request.

Dear Dr Lions, my deepest apologies for not attending, it was out of my hands, I'm actually at the hospital now as a medical patient regarding a boar attack which stopped in the way of me being there, if you passed by one of rooms in the hospital I'm sure you would've seen me, I'd like to reschedule for sure, please let me know when you'd be free.
